Ohio’s Medical Marijuana Advisory Board has set up an on-line process to obtain your patient ID card. This will be done at your recommending doctor’s office..
First, your doctor must have a “Certificate to Recommend”. While this is not difficult to obtain, your doctor may have questions on how to do this. We have another step by step guide to show how this is done. You must have a bona-fide doctor patient relationship with the recommending physician and your doctor must submit your application for you. This is different than other states where you obtain a document from your physician and submit the information to the state yourself. Note than only physicians’ licensed and practicing in the state of Ohio can recommend for you. In other words you can’t have a Pennsylvania physician (even though they may be certified to recommend cannabis in that state) submit your recommendation for you.
Your doctor must diagnose you with one of the current qualifying conditions for marijuana in Ohio and your doctor must submit this diagnosis to the state board of pharmacy within 90 days for the recommendation to be valid.
Fees: The annual fee for an Ohio patient ID card is $50. You will pay this after your recommending doctor sends in your registration application on-line. Most likely you will receive a registration email with a link to pay your fee. Caregivers will pay a $15 annual fee and veterans and indigent patients may pay a reduced fee.
Note the board requires a patient’s proof of residency (regardless of age) which could be one of the following:
- A valid Ohio driver’s license
- A valid Ohio identification card (issued through the state’s Bureau of Motor Vehicles (BMV), or
- A valid U.S. passport or passport card.
So in summary the following items must be submitted by the recommending physician to obtain a medical marijuana ID card for the patient.
- Physician Recommendation Form
- Application Fees
- Proof of residency
This uploaded information is then reviewed by the board and if all information is correct, a medical marijuana card is issued to the patient. The card is good for a year and expires on the last day of the month the card was issued. The board will notify you 45 days before the card is set to expire.

- Multiple Sclerosis
- Pain that is either chronic and severe or intractable
- Parkinson’s Disease
- HIV (positive status)

- sickle cell anemia
- spinal cord disease or injury
- Tourette’s syndrome>

- ulcerative colitis
